If I am a transfer student do I still have to have a Health History form completed?

All undergraduate and graduate first-year (Pharmacy/PGx/MAT programs), and transfer students are required to submit a Health History form.

When is my health history form due?
Health forms are due by August 1st.

What if my physical date is scheduled after August 1?
You need to notify your Athletic Trainer.

Where do I send the health history form?
You can upload your completed forms onto your Med+Proctor account.

Can I have over the counter and/or prescription medications in my dorm room?
Yes, you can keep all personal medications in your dorm. If you need Depo Provera or Allergy injections, a signed provider prescription is required, and medications can be kept in Health Services. *
*Students are responsible for scheduling their own appointments for each injection.

Can I have my Tb test administered or read at Health Services?
No, you will need to visit a local clinic or a medical laboratory to have services completed.
